life-limiting diagnosis for you
or a loved one does not need to
mean pain and hopelessness.
Our hospice care services provide a specialized program of care provided for patients and their families who are facing life limiting illnesses.
The philosophy is to relieve pain and other symptoms associated with life-limiting illness. Based primarily in the home, provided services include medically-directed, nurse-coordinated care and support, enabling families to remain together in peace, comfort, and dignity.
Examples of life-limiting illnesses: Cancer, Congestive Heart Failure (CHF), Stroke (CVA), Pulmonary Diseases (COPD), Renal Disease, Liver Disease, Dementia/Alzheimer’s Disease, AIDS, Failure to Thrive, Debility Unspecified, ALS (Lou Gehrig’s disease), and Parkinson’s disease.
Hospice care is designed to
benefit everyone - from the
patient to family members and
friends. This is achieved by
determining the needs of each
individual served by the Hospice
team, carefully assessing the
available alternatives, and
encouraging them to make
decisions that will best meet
their needs.
